Community Outreach

Helps communities function better and help break down barriers by assessing their needs and guiding them to the appropriate resources.

COMMUNITY HEALTH WORKER (PROMOTORA) PROGRAM

CHWs or Promotoras de Salud are trusted members of the community that share many of the same social, cultural and economic characteristics.  They are a vital component in linking patients to community resources and medical services to achieve positive changes and good health. As part of the medical team, they help patients better manage their chronic conditions.

Promotoras work with multiple community partners to identify and facilitate services for Migrant Seasonal Agricultural Worker Families

The Promotoras (Community Health Workers) serve as:
  • Advocates

  • Mentors
